Order Via FAMIS
- Log on to the FAMIS website: https://dfoforms.nycenet.edu/ecatalog/default.aspx
- Search via our Vendor name: Examgen or our contract number: 7000636
Order Via Non Contracted Vendor Way
- Log into NYC FAMIS Purchasing as usual, then, under “Options,” select “Non-Contracted”; “Commodities,” then “Sole-Vendor.”
- When prompted, select “Vendor Inquiry” and enter our vendor ID (EXA050000) or our Name (Examgen). Select Examgen from the list.
- Fill out the Delivery, Invoice and Account information as needed. Then enter the items you wish to purchase, attach a Sole-Source Letter and W-9 Form, and click on Submit Order. For your convenience, we have electronic versions of our Sole-Source Letter and W-9 Forms available here:
